I will need the following information before our appointment so I can pull your chart and have it ready. Your name, entire birthdate, the city & state where you were born as well as your birth time. If you do not have your birth time, we can still pull a chart. However, I do prefer having the birth time if possible, as it will reflect the most accurate information.
